UT-NDPA-V1 (with No Exhibit) Utah's version of the National Data Privacy agreement with no Exhibit. Current DPA version as of November 2020.
UT-NDPA-V1 (with Exhibit) Utah's version of the National Data Privacy agreement. Current DPA version as of November 2020.
UT NDPA Model Email Language This model email language can be copied and pasted into a email body with the UT NDPA attached when emailing vendors.

Declined No agreement reached with vendor, application not in use.
District Modified District has slightly modified V1 or V2 agreement with a particular vendor.
EISP Originating NDPA with Exhibit H An EISP - Vendor Specific (UT-NDPA-V1) with Exhibit should have specific language in Exhibit H about sharing data with a program evaluator. The Early Intervention Software Program (EISP), established by the Utah State Legislature and administered by the Utah State Board of Education (USBE), provides Utah’s Local Education Agencies (LEAs) with the opportunity to select adaptive computer-based literacy software programs. The goal of this program is to increase literacy outcomes for kindergarten through grade 3 students.

In Use/iKeepsafe Badge iKeepSafe certifies technology used by children and in educational settings with qualified experts.
In Use/Privacy Pledge K-12 school service providers are honored to be entrusted by educators and families to support their educational needs and school operations. School service providers take responsibility to both support the effective use of student information and safeguard student privacy and information security.
Legal Drafted Guardian Release Parent release obtained for each student's use of the application.

Self-Hosted No agreement is in place since the application is hosted by the district.
STEM Action Center Agreement The STEM Action Center is Utah’s partner in promoting Science, Technology, Engineering and Math education through the identification and support of best practices and leveraging of resources across education, industry, government and community partners to support economic prosperity.
UETN Statewide Agreement An agreement entered by UETN on behalf of LEAs. This agreement type will only be used by UETN to show their resources.
USBE Statewide Agreement An agreement entered by USBE and a vendor on behalf of Utah LEAs. This agreement type will only be used by USBE to show their resources. This agreement type will show resources that LEAs do not need to cover privacy provisions for as they have already been covered in the USBE Statewide Agreement.
Vendor-Specific Modified version of the standard agreement that is strictly used with a specific vendor.
